With the intensification of population aging, the health status of the elderly has drawn significant attention, yet many individuals' medical needs remain unmet. Data indicates that over 60% of seniors delay seeking medical care due to “fear of troubling their children” or “believing the issue is not serious,” leading to minor illnesses progressing into severe conditions. Additionally, some suffer from geriatric syndromes such as polypharmacy and malnutrition due to a lack of systematic management.

The Department of Geriatric Medicine at HeYou Hospital has launched the “Free Geriatric Health Status Assessment” initiative, aiming to promote the concept of “early screening and early prevention” for geriatric health risks, thereby reducing the incidence of major risks such as disability and falls.

II. Full-Cycle Geriatric Health Management System

The global aging health crisis is significant. Among individuals aged 65 and above, 42% have at least three chronic conditions concurrently (data from the National Health Commission); 30% of early symptoms of dementia are mistaken for “normal aging,” missing the golden window for intervention (The Lancet study); falls are the leading cause of injury-related death among the elderly, with 50% of those who fall requiring hospitalization (WHO statistics).

Through the Geriatric Health Status Assessment, this initiative will establish a full-cycle geriatric health management system of “screening-intervention-follow-up,” enabling multidisciplinary collaboration (e.g., nutrition, psychology, rehabilitation) in diagnosis and treatment to safeguard the health of the elderly population.

1. Medical Intervention: Rapid Identification of High-Risk Groups:

✓ Potential Malnutrition Group (dietary assessment below standard)

✓ High-Risk Polypharmacy Group (taking 5 or more medications simultaneously)

2. Chronic Disease Population Management, e.g.:
