HeYou Hospital's "Birth Medical Certificate" Issuance Time Adjusted, Don't Make a Trip for Nothing!

Attention, parents! The issuance time of the "Birth Medical Certificate" at HeYou Hospital has been adjusted. Please save the following information!

I. Application Methods and Times:

1. Offline Application: Make an appointment for in-person processing via the "Yue Sheng Shi" mini-program.

On-site issuance time for the Birth Medical Certificate at HeYou Hospital:

Every Wednesday, 8:00-12:00, 14:00-17:00

2. Online Application: Request mailing via the "Yue Sheng Shi" mini-program.

One week after the newborn's birth, the mother and father must complete identity verification, information review, upload photos of their valid ID documents, submit mailing information, and confirm with an electronic signature. The birthing institution will then review the relevant information and issue the Birth Medical Certificate.

The Birth Medical Certificate will be sent to your home via EMS (cash on delivery) every Wednesday afternoon.

Note: The Birth Medical Certificate must be collected from the hospital where the delivery took place and cannot be exchanged at a different location.

II. Important Notes for Applying for the "Birth Medical Certificate":

(1) Adhere to the real-name delivery system. Applicants must provide the original and a copy of the newborn's parents' valid identification documents. If the ID card is expired or invalid, please go to the public security authorities to replace it with a valid second-generation resident ID card before proceeding.

(2) Prepare a suitable name for the newborn in advance. The newborn's name must use standard Chinese characters as stipulated by the state; traditional or rare characters are not permitted. Once the "Birth Medical Certificate" is issued, the information on the certificate will generally not be changed.

(3) Carefully verify the application information, as the content cannot be altered after printing. Please carefully check the parent information, newborn's name, gender, and other details on the First Issuance Registration Form and the Application Information Registration Form for the "Birth Medical Certificate".

(4) The "Birth Medical Certificate" should generally be applied for by the newborn's mother. If the applicant is not the mother herself, a "Power of Attorney for Obtaining the Birth Medical Certificate" signed by the mother, along with the original valid ID documents of both parents and the applicant, must be provided.

(5) The "Birth Medical Certificate" must be kept properly. It is one of the essential documents for registering household registration and nationality, enrolling in school, and obtaining social security for the newborn.

III. Important Notes for Keeping the "Birth Medical Certificate":

(1) Do not alter it privately. The "Birth Medical Certificate" is an important legal document. Please do not draw, write, or mark on it.

(2) Avoid loss or damage. Keep it safe from theft, loss, and moisture. Do not cut or separate it.

(3) Do not laminate or fold it. The "Birth Medical Certificate" has optical watermark anti-counterfeiting features. Laminating or folding can easily affect the identification of these features.

Tips for Exchanging the "Birth Medical Certificate": You need to provide: ① The original Birth Medical Certificate; ② Original and copies of both parents' ID cards and household registration books; ③ If the newborn has been registered, provide a copy of the newborn's household registration page; ④ The reason for exchanging the original Birth Medical Certificate. An application must be submitted to the birthing hospital, and exchange is only possible after hospital approval. The process is time-consuming.

Tips for Re-issuing the "Birth Medical Certificate": The re-issuance process is similar to the exchange process, but after the birthing hospital's approval process is complete, the re-issuance must be done at the Shunde District Maternal and Child Health Hospital (approximately 15 km from HeYou Hospital).
