A 14-Hour Flight to Show Off Her New Knee: A Chinese American Patient Returns Across the Pacific for Follow-Up Care

"I never imagined I'd fly 14 hours back a year later, just to visit my old friends."

On a spring morning in April, the Department of Joint Surgery & Sports Medicine at Heyou Hospital welcomed back a familiar face. More than a year earlier, Ms. Zhang (pseudonym), a Chinese American in her seventies, had traveled from Los Angeles to Shunde for knee replacement surgery. This time, she returned not because something was wrong, but for a routine follow-up, and to proudly show her care team how well her new knee was doing.

Her story offers more than a successful surgical outcome. It also highlights an often-overlooked aspect of joint replacement: the importance of long-term bone health management after surgery.

A Journey Across the Pacific for the Right Care

After living in Los Angeles for many years, Ms. Zhang found that pain in both knees had gradually progressed from discomfort while climbing stairs to sharp pain even when walking on level ground. The pain eventually became severe enough to disrupt her sleep. She consulted multiple physicians in the US. Some recommended conservative treatment, while others advised surgery. Regardless of the approach, she faced lengthy waiting times, high medical costs, and concerns about managing postoperative recovery without an integrated rehabilitation program or family support nearby.

During her visit, she tried the department's anti-gravity treadmill, which allows patients to walk with reduced body weight and minimal stress on their joints. "It was amazing," she said. Follow-up X-rays showed excellent results. Her knee implant remained well positioned with no signs of loosening, and the surrounding bone density had been well maintained.

Calcium Matters After Knee Replacement

Many patients ask, "Doctor, my knee has been fixed. Why do I still need calcium?"

It's a common question, and the answer is an important one.

Preventing implant loosening: Knee replacement surgery relies on implants that must remain securely anchored in healthy bone. When osteoporosis weakens the surrounding bone, the implant may gradually become unstable, leading to micromotion, subsidence, and ultimately implant failure.

Reducing the risk of postoperative fractures: Older adults are already at increased risk for osteoporosis. If a fall occurs after surgery, weakened bone around the implant is more susceptible to fracture, one of the most serious complications following joint replacement.

The goal of calcium supplementation after knee replacement isn't simply to support the joint itself. It's to strengthen the bone that supports the joint.

A Two-Pronged Approach to Bone Health:
More Than Replacing Calcium, It's about Preserving It.

Professor Wang attributes Ms. Zhang's excellent bone health to her consistent adherence to a treatment strategy designed to both reduce bone loss and improve calcium absorption.

1. Reducing Bone Loss: Protecting Bone From Within

Bone is constantly being remodeled. After surgery, reduced physical activity may increase bone resorption, accelerating calcium loss.

Oral medications: Bisphosphonates, such as alendronate, taken once a week, can help slow bone resorption.

Intravenous therapy: For patients who cannot tolerate oral medication or have difficulty maintaining long-term treatment, intravenous zoledronic acid administered every six to twelve months may provide sustained protection against bone loss.

2. Improving Calcium Absorption: Building Stronger Bone

Preventing calcium loss is only part of the equation. The body also needs to absorb and incorporate calcium efficiently into bone.

Calcitriol: As the active form of vitamin D, calcitriol enhances intestinal calcium absorption and promotes bone mineralization.

Denosumab: This anti-RANKL monoclonal antibody is administered by subcutaneous injection every six months. It suppresses the formation and activity of osteoclasts and is among the most effective therapies for reducing bone resorption. When used together with calcitriol, the two therapies complement one another, one improving calcium absorption and the other helping preserve it within the skeleton.

Ms. Zhang's Daily Bone Health Routine

In addition to following her prescribed medications, Ms. Zhang has incorporated bone health into her everyday life.

Nutrition: One glass of milk each day, soy products, deep-sea fish, leafy green vegetables, and a reduced-sodium diet (since excessive salt intake can accelerate calcium loss).

Rehabilitation: Regular non-weight-bearing exercises, including straight-leg raises and ankle pump exercises.

Medications: Daily supplementation with 500 to 600 mg of elemental calcium together with calcitriol; a weekly antiresorptive medication; and a denosumab injection every six months after surgery.

"Those small daily habits really do make a difference," Ms. Zhang said. "Now I can comfortably walk five or six thousand steps a day and even do my own grocery shopping."
