Managing comorbidities is not a simple superposition of treatment plans for individual diseases; rather, it involves developing continuous, comprehensive, and individualized diagnostic and treatment strategies centered on the specificities of geriatric diseases, multimorbidity, geriatric syndromes, and polypharmacy.

Conduct detailed screening and assessment for geriatric syndromes (such as Alzheimer's disease, dementia, falls, sarcopenia, frailty, visual and hearing decline), and implement personalized intervention plans based on the assessment results, including pharmacological and non-pharmacological treatments (exercise rehabilitation, nutritional therapy, psychological support, and social support).

Pathophysiological changes associated with aging lead to specific disease manifestations in the elderly. Based on these specificities, formulate precise and individualized diagnostic and treatment plans, implementing targeted management strategies.

Specializing in combined cardiopulmonary rehabilitation, focusing on balance, gait, and coordination in the elderly, with emphasis on medical assessment, physical/occupational therapy, and cardiopulmonary function training. Integrate psychological, traditional Chinese medicine, and nutritional support, and assist with the management of pressure ulcers, pain, and spasticity, to maximally restore the daily living abilities of the frail elderly and those disabled by illness or injury, improve self-care capacity, and reduce the incidence of prolonged bed rest and senile dementia.

Centered on the "three highs and two diseases" (hypertension, hyperglycemia, hyperlipidemia, coronary heart disease, stroke), implement integrated, multi-dimensional chronic disease management for the elderly. Maintain functional status, provide long-term holistic and continuous diagnosis, treatment, rehabilitation, and care, to enhance the living abilities and improve the quality of life of the elderly.
